Wang Jian Clinical Laboratory
From April to t July 2018, I studied at the KHR Hospital in Vienna of Austria for three months. Thanks to the hospital for providing me with this valuable learning opportunity. I would like to thank the leaders and teachers of the hospital for their great assistance. My learning experience is summarized as follows.
1. Austrian profile
Austria is a landlocked country in central Europe. The terrain is high in the west and low in the east. The Alps run through the west and south of Austria. The northeast is the Vienna Basin, and the southeast and north are hilly lands and plateaus. The Danube River flows through the northeast and is 350 kilometers long in the territory. Most of Austria is in a maritime climate and continental climate transition zone with cold winters and cool summers. The city of Vienna is the capital of Austria. Due to the strong atmosphere of classical music in the city, musicians from all over the world gather here, and it has the reputation of “world music capital”.
2. Austrian social security situation
In Austria, there is a comprehensive social security and social care system. This system is embodied in two aspects. The first is the insurance principle. According to this principle, all practitioners and family members have social security after illness, accident, unemployment, maternity leave and retirement. Second is the federal, state and administrative district public relief service to citizens whose survival is not guaranteed through the insurance system. The cost of the insurance system is borne by the society.
Austrian clinics and hospitals are distributed and cover throughout the country. The principle of compulsory insurance plus insurance for children and non-employed spouses guarantees that almost all the population can enjoy the corresponding insurance protection. In addition, there is also private sickness insurance that provides additional services. The medical expenses of the poor are borne by society. According to the experts, there is a general awareness of self-prevention among the general population, and as a result, more and more Austrians are asking for health checks paid by disease insurance companies. Perfect medical security has laid a solid foundation for a good doctor-patient relationship.
3. KHR Hospital
KHR Hospital is the second largest hospital in Vienna. The hospital has a history of more than 100 years. It consists of two districts. The annual emergency volume is about 300,000. The number of annual inpatients is 53,000, and it consists of 20 clinical departments and 8 research institutes with a total of 3,500 employees. .
KHR hospital has a beautiful environment and a large green area. The lawns and flowers are all over the hospital. It is very quiet. There are small animals such as squirrels and various birds in the courtyard, like a beautiful forest park.
4. KHR Hospital Learning Experience
Strict and complete quality management system
The quality management of the laboratory is very strict and the Austrians work very rigorously. Like a regular project, they do quality control in 4 hours. Special items, if there are specimens on the day, you should first do the quality control. In the case of reagent replacement, it is necessary to perform quality control for verification. The calibrator and control materials are placed neatly on the table for the operator to use. It has invested a lot of people, time and economic costs in quality management, and it also reflects the superiority of its medical security system.
Report management system for layer-by-layer review
Although the hospital's specimen volume is smaller than ours, the report review is very strict. A report is issued and requires three levels of review. The machine operator conducts the first audit, and the team technician performs the second audit and sends it to the test physician workstation for a third audit by the inspector in the department, and then to the clinical workstation. For reports of abnormal results, the testing physician will recommend further tests on the report. After the clinician agrees to apply, the inspecting physician will conduct a detailed analysis on the second report to assist the clinician in the diagnosis and treatment. Although the clinician did not understand some of the test items, the project was fully utilized with the assistance of the test physician.
Advanced and humanized medical equipment
The laboratory's equipment is very advanced, and it also has a lot of humanized small equipment, replacing most of the manual operations. It not only reduces the burden on employees, but also is easy to reduces manual errors.
Post-inspection treatment of specimens
After the inspection, the specimen will be automatically sealed by the machine, scanned, placed on the shelf, and the information will enter the computer system. To query a specimen, you need simply enter patient information on your computer.
Harmonious doctor-patient relationship
The patient comes to the first floor of the clinic and waited quietly in the waiting room. The doctors and patients will have a cordial conversation and the relationship is very harmonious. The patient has great trust and respect for the doctor. The entire medical environment remained quiet, there is no loud noise, and no doctor-patient disputes occur.
Complete detailed diagnostic report
The inspection doctor's report on the abnormality will be written on the report after the internal review, and further inspection will be submitted. Clinicians will take these comments seriously and apply for further in-depth inspections. The doctor will analyze the second report. For example, patients with clinical coagulopathy which cannot be diagnosed will be tested for all clotting factors with detailed diagnostic analysis.
Respect and protection of privacy
Information with the patient's name and identity is strictly prohibited from being photographed and leaked. In the department, it is not allowed to have a photo taken by the staff. Paper with patient information is placed separately in a special trash can. All these measures reflect the respect and protection of Austrians' personal privacy.
5. Austrian life perception
The Austrians are of high quality and strictly abide by social orders. They line up to wait for the subway. The subway cars are always keeping quiet, even if people are speaking, they talk in quiet tones. In the station escalators, people always wait in the right row and leave the left side for emergency people. Garbage is strictly classified, and paper, metal, glass, and household garbage must be placed in different trash cans. These are all worth learning.
After these three months of study, I am not only impressed by the enthusiasm and sincerity of the Austrians, but also experienced their rigor and passion. In terms of quality management and professional knowledge, it has widened my horizon and improved my professional understanding. Thanks to Professor Koller, Doctor Walletin, and others who helped me during the learning process.
